(Page 2)
MINE ID # OR PROSPECTING nn_~gg~_nng
INSPECTION DATE lningm~ INSPECTOR KAP
OBSERVATIONS
1. This inspection was conducted as part of the Division of Minerals and Geology's monitoring program.
Present at the inspection were Kate Pickford, DMG and Troy Pelton, White River Aggregates, Inc. Weather at
the time of the inspection was clear, calm, and approximately 70 degrees.
2. A sign was posted at the entrance to the site and the boundaries were marked. A stockpile of topsoil for
reclamation is located along the top of the highwall. The stockpile, however, is poorly vegetated and does not
fulfill the requirements set forth in 3.1.9 (1) of the Constructions Materials Rules and Regulations.
3. An area in the bottom of the pit used for washing material contains a considerable amount of sediment. Mr.
Pelton advised that this wash pond is cleaned of sediments annually. Runoff at the site appears to be
contained within the permit boundaries. Noxious weeds were not present at the site.
4. All activities and completed work at the time of inspection appeared to be following the guidelines set forth in
the mining plan for the pit. A bond recalculation will be conducted as a portion of this inspection in order to
determine whether the current bond is adequate to reclaim the site.
I & E Contact Address
NAME Mr. Jack E Paulson
OPERATOR: White River Aggregates, Inc.
STREET: 1107 E.
