(Page 21
r
MINE ID rr OR PROSPECTING ID # M-94-005
INSPECTION DATE 4 2/ 1/99 INSPECTOR'S INITIALS RCO
OBSERVATIONS
This inspection was performed by the Division as part of its monitoring of Construction Materials 112 permits. The
operator was not contacted about the specific inspection date, but the inspection topics were discussed two weeks prior
to the actual inspection. This partial inspection was performed by observing onsite features from outside the permit area
boundary, and did not necessarily need to involve the operator.
The permit ID sign was observed as posted at the entrance to the site off Highway 550.
The topsoil which is stockpiled along the northern, western and southeastern boundaries was the primary topic inspected.
Those topsoil piles have been inadequately vegetated, and this item was brought to the attention of the operator during
a recent telephone conversation. A verbal commitment was made to attempt to revegetate them by hydroseeding them.
The hydroseed/hydromulch application has apparently been performed. All of the topsoil berms named above show
evidence of a recent application.
The piles should be monitored in the coming seasons for revegetation success, and re-applications made if necessary. (The
young trees along the north side should likewise be regularly monitored and replaced in a timely manner if necessary.)
The operator stated during the same recent phone conversation that there will be several areas which will be available to
reclaim in the northern end of the permit area, probably in 1999. Please ensure that the approved reclamation plan is
reviewed and followed. All reclamation carried out should be clearly reported in the annual report, with a map if applicable.
There were no problems observed at this time. The next full inspection and/or bond recalculation will probably be scheduled
for year 2000 or later.
For all questions or responses to this report, please contact this inspector at the following address or phone: 484 Turner
Dr., Bldg.
