The following problems (PB) and/or possible violations (PV) (and suggested corrective
actions) were identified during this inspection. The problems should be corrected by the
dates given, or they will become possible violations. The possible violations should be
corrected by the dates given to reduce their severity when considered by the Mined Land
Reclamation Board. The inspector noted on the previous page should be notified of all
corrective actions taken.

1. There was not a mine identification sign posted at the 01/31/97
public entrance to the mine site ae required by Rule PB or PV: PB
3.1.12(1)(a),(b) & (c), and the permit boundaries were TOPIC(S): SM
not marked by monuments or other markers as required by
Rule 3.1.12(2)
Corrective actions are to:
1) Place a mine identification sign at
the entrance to the mine site that
contains at a minimum: the name of
the operator and "NLRB Permit / M-81-
138".
2) Install permit boundary markers or
monuments at each corner of the
approved permit area.
3) Submit a photograph to our office
which clearly shows that the
identification sign and permit
boundary markers were installed.
