<br />MINE ID f OR PROSPECTING ID <br />INSPECTION DATE 10/10/96 <br />(Page 3) <br />K-e1-138 <br />INSPECTOR'S INITIALS AJW <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There was not a mine identification sign posted at the 01/31/97 <br />public entrance to the mine site ae required by Rule PB or PV: PB <br />3.1.12(1)(a),(b) & (c), and the permit boundaries were TOPIC(S): SM <br />not marked by monuments or other markers as required by <br />Rule 3.1.12(2) <br />Corrective actions are to: <br />1) Place a mine identification sign at <br /> the entrance to the mine site that <br /> contains at a minimum: the name of <br /> the operator and "NLRB Permit / M-81- <br /> 138". <br />2) Install permit boundary markers or <br /> monuments at each corner of the <br /> approved permit area. <br />3) Submit a photograph to our office <br /> which clearly shows that the <br /> identification sign and permit <br /> boundary markers were installed. <br />
