• (Page 21 <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 6/26/97 <br />M-78-222 <br />OBSERVATIONS <br />• <br />INSPECTOR'S INITIALS WHE <br />This partial inspection of the Monogram Mines was conducted as required by DMG routine <br />monitoring inspection program. Operator was contacted prior to the inspection but could not <br />participate due to short notice and conflicting schedules. Operation is approved for surface and <br />underground extraction of uranium and vanadium ores, 9.9 acres affected area. DMG holds S8,848 <br />financial warranty. No activity was observed at the mine site during the time of this inspection, <br />operator has commenced final reclamation. <br />Signs & Markers -Mine identification signs, required by Rule 3.1.1211), were not observed. <br />Affected area perimeter markers, required by Rule 3.1.1212), were not observed. The separate <br />mining locations approved under the Monogram Mines permit are located in close proximity to <br />numerous pre-law mining disturbances. Given the absent signs and markers, DMG inspector had <br />difficulty identifying permit areas and delineating permitted activities from pre-law disturbances. <br />Monogram Pit -Map submitted by the operator, titled, "Monogram Mines, Reclamation Work <br />Completed in 1995", dated 1 1 /20/95, indicate that highwalls had been reduced, vent holes <br />backfilled, portals closed, and affected lands recontoured for this 0.8 acres area. However, DMG <br />inspector located an open pit with near vertical inslopes and an open portal near the approximate <br />location of the Monogram Pit, possibly pre-law. Please clarify. <br />Great Hesper Pit - Highwalls had been reduced, portions of backfilled highwalls ranged from 3H:1 V <br />to 1 H:1 V slope. <br />Buckhorn Mine -Not inspected. <br />Canopus Portals -Southern portal of the three was inspected. Portal had been backfilled, waste <br />rock stockpile graded to blend with surrounding topography. Vegetative cover included Indian <br />ricegrass, white clover and foxtail barley. <br />Capella Mine - Stormwater containment berms were intact on top of waste rock stockpile. <br />Collapsed portal decline was not closed or backfilled.
