(Page 2) <br />MINE ID# OR PROSPECTING ID# M-1980-131 INSPECTION DATE 08/04j04 INSPECTOR'S INITIALS GRS <br />This was a general monitoring inspection in response to Lafarge's notification to the DMG <br />that they had completed the work on the inlet and outlet structures approved by Technical <br />Revision 001, approved on February 9, 2004. <br />The permit area is current being mined by Elam Construction, Inc., under an agreement with <br />Lafarge. Although there is little excavation taking place, there are several dozen <br />stockpiles of materials located on the site. These materials are being processed and removed <br />from the site. The. area of the inlet and outlet structures is to the south and east of the <br />active mining and processing sites. The structures are located on a large mined out pit <br />which is mined out and reclaimed. The reclaimed pit area is well graded and sloped and has a <br />good cover of mature grasses and woody plants on the slopes and adjacent banks. Because of a <br />high water condition during the spring of 2003, flood water had entered the pit from the <br />Yampa River and then cut an exit channel where it left the pit. The operator has constructed <br />inlet and outlet structures to prevent any further damage to the pit walls or adjacent <br />property. The structures were installed with an underlying filter cloth and durable rock rip <br />rap. The work appears to have been well done and adequate to satisfy the concerns of the <br />DMG. <br />A Permit ID sign was posted at the site, because of the location it was a little difficult to <br />see from the entrance road, the permittee's representative stated that he would have the sign <br />moved to a more visible location. The permit boundaries can be identified by the fences <br />located adjacent to the site.
