ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ti+-Iean-red <br />INSPECTION DATE: L(1/Od/Od INSPECTOROS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of <br />the site on 10/6/04.Drain cover deployment is completed, in the Phase IVC <br />area. The financial warranty for the phased area is already in place. CC&V <br />plans to load the pad prior to the submittal of the certification report, <br />but will not apply solution until the report is submitted to the Division. <br />The certificate report is going to be submitted the week of the 11th. Main <br />purpose of the inspection was to collect the 4th quarter samples from the <br />under drain discharge area pipes. Lawrence Meyers of CC&V was present <br />during the inspection. Inspected the under Darin areas, AG 1.5 was dry, the <br />west side surface water diversion sump was wet, no measurable water noted. <br />The lowest under drain pond was holding a very small amount of water; both <br />the upper two ponds were dry. Samples were collected from the South under <br />drain and 4 inch pipes for wad cyanide analysis. Operator took a split <br />sample using Division provided bottles and their own. Samples were placed <br />in cooler with ice and delivered to CDOH&E lab in the afternoon. <br />Inspected the Phase I, II, III AND IV pregnant solution collection areas <br />and recorded the following readings from the high and Low volume solution <br />collection transducers. <br />Phase I High volume solution collection transducer # 1 was recorded at 51.8 <br />#2 was at50.8 and # 3 was at 51.9. Low volume # 1 was at.22 and # 2 was at <br />.32. Piezometer for the pond was at 48.6 FEET <br />PHASE II AND III High Volume solution collection transducer # 4 was at 37.1 <br /># 5 was at 37.7 and # 6 was at 39,0 Low volume transducer # 1 was recorded <br />at .12 and # 2 was at .42. Piezometer was at 38.5 feet. <br />Phase IV High and Low Volume solution collection transducer # 7 was <br />recorded at 34.8 # 8 was at 34.0 # 9 was at 35.0 #10 was at35.2 Low volume <br />solution collection transducer # 1 was recorded at 13.9 and # 2 was at 7,6 <br />inches of hydrostatic head. (ALL OTHER READINGS ARE IN FEET OF HYDROSTATIC <br />HEAD UNLESS NOTED) <br />
