(Page 2j <br />MINE ID # OR PROSPECTING ID # M-2001-113 <br />fNSPECTION DATE 3/13/02 INSPECTOR'S fNITIALS RCO <br />OBSERVATIONS <br />This was apre-operation inspection performed by the Division on the site of a new 1 12 permit application, The operator <br />was contacted about the inspection and was present throughout the inspection. Also present throughout the inspection <br />was the landowner, Mr. Dana Gately. Prior to performing the inspection, all parties present met at the operator's office <br />to briefly review the most current adequacy response narratives and maps, which were hand delivered to thgis inspector <br />that day. Snow cover was thin and covered only a fraction of the site, which allowed adequate observation of the site. <br />The operation is to consist of three mining phases, which are clearly separated on the ground by the existing dirt road, <br />whichg will provide access to the phases, and by the existing irrigation ditch which will be rerouted when mining intercepts <br />it. All activity will be south of Tomichi Creek on land owned by Mr, Gately. <br />There is some pre-existing construction-related disturbance on the northern phase of the site. There are a few pieces of <br />heavy equipment, equipment parts and debris from construction jobs. There is a small pit on the site, in which water was <br />impounded. Several existing fences exist on the boundary and inside this phase. <br />The southern two phases are currently still haymeadows. There are some fences, willow thickets, ditches and swales on <br />the site. The site has been mowed, but there is no additional evidence of any activity. In many places the topsoil is fairly <br />thin, and the alluvial gravel is exposed. <br />No roadway or bridge upgrades appear to be needed, since they are presently traversed by heavy equipment associated <br />with access to a nearby permitted pit IM-1996-0441, <br />There were no additional items observed or discussed.
