The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE 1. Records indicate this site should be either intermittent or in Temporary Cessation. Past re nests 11 / 02/03 from the Division to change the status have pone unanswered. PB or PV: ~~ TOPIC(S): ~B. Corrective Action: - - - (1)Submit aTechnical Revision to the permit to chance the status to intermittent status pear CRS 34- 32.5-103(b) or (2) submit {photo documentation that mining has resumed (3) Provide photo documentation that total reclamation of the site has begun or (4) request a Board Hearing for the orantino of a second Temporary Cessation as is required under Rule 1.13.5(3) by the Corrective date noted. 2.
