(Page 2) <br />MINE ID # OR PROSPECTING ID: # M-1978-287 <br />INSPECTION DATE: 9/5/03 <br />INSPECTOR'S INITIALS GRM <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Minerals and Geology. The Vader Pit is located approximately 4 %2 miles east of G7.umison south off US 50. <br />The inspector contacted the operator, but he did not accompany on the visit. <br />The mine identification sign and affected azea boundary markers are in place and incompliance with Rule 3.1.12. <br />The sign was located at the main entrance to the site, although covered by weeds. The: boundary mazkers consisted <br />offence lines and other distinguishable markers. <br />The site appears not to have been active for sometime. Annual reports and the inspection from 2000 note the lack of <br />activity within the permitted area. A note from the 2000 inspection report states that if no activity was reported <br />within the next annual report, staff would be contacting the operator regarding the status of this permit. Per Rule <br />1.13, this site appears to have been in temporary cessation since at least 1995 or at the least intermittently used. <br />Due to the lack of activity reported or observed and several requests from this Office for the operator to <br />review the status of this permit, this Office is listine the inactivity as a aroblem under Records with a <br />corrective action and date noted at the end of this report. A first, temporary cessation Date of 10/4/1996 has <br />been established based on Annual Reports. Failure to complywith the corrective action may result in the issue being <br />changed from a problem to a Possible Violation, which will include a Boazd Hearing, possible civil penalties and / <br />or the revocation of this permit. <br />Three stockpiles of materials were noted on site. The weed growth on these processed materials indicates theyhave <br />not been utilized recently. The gravel work pad was also noted as being heavily invaded by annual weeds that <br />although not noxious show a lack ofproper maintenance. Per Rule 3.1.10(6), weed control shall be employed ifthe <br />weed species threaten to continue the development of other vegetation. The operator also has a responsibility to <br />under CRS 34-32.5-116(e} to control unsightliness or deleterious effects to the site caused by the mining operation. <br />The lack of site maintenance is listed as a aroblem under Revegetation with a corrective action and date <br />noted. <br />No equipment was noted on site. There was no observed storage of fuels or other hazazdous materials on site. <br />The inspector observed no off-site disturbance, erosion, sedimentation or storm water issues. <br />The General Mine Plan appears to have been followed with the noted lack of activity. The Reclamation Plan has <br />not been implemented to date to any major degree. Staff cannot verify slopes meet the requirements within the pond <br />area. This is not an issue right now, but will have a major impact on any release requests in the future. If low water <br />affords an opportunity to photograph the slopes below the normal water line, staff strongly encourages the operator <br />to document the slope angles. <br />
