The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE The permit boundary is not adequately marked, and there is no permit ID sign, though it 6/1/06 is required under Rule 3.1.12. PB or PV: PB TOPIC(S): SM CORRECTIVE ACTION: The operator must locate and mark the permit boundary, and _, _, post an ID sign at the entrance(s). Proof of performing these actions must be received at this office by the date shown at right. The annual report map is inadequate (out-of-date, not to scale) and needs to be 6/1/06 replaced. PB or PV: PB TOPIC(S): AR CORRECTIVE ACTION: The operator must prepare and submit an adequate map of _, _, the site, to be used for future annual reports (see map standards under Rule 6.2).
