(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2000-049 <br />INSPECTION DATE 6-6-2003 <br />INSPECTOR'S INITIALS SSS <br />this was apre-operational inspection of the acreage added to Phase I of the permit area through the Amendment No. 1 <br />application process. <br />The clay liner and riprap placement in Gurley ditch has been completed and looks stable <br />There are no signs of seepage on the 30 foot pit highwall nearest Gurley Ditch, indicating that the liner project appears to be <br />effective in minimizing seepage from the ditch in the Phase I area. <br />The settling pond and wash plant facility, proposed by Amendment No. 1 was in place and a test run had just been completed. <br />The operator is reminded that these facilities are not to be installed before DMG approval of such facilities. Future occurrence <br />of this nature may lead to an enforcement action. <br />The wash plant settling basin is full of water that appears to be exposed groundwater. If this is the case, the operator must fill <br />in this pond, cover it with 2 feet of material, and create the settling pond above this grade. Please clarify the origin of the water <br />in the settling pond and, if it is groundwater, fill in the pond, cover itwith a minimum 2 feet of material, and submit photographic <br />documentation of this corrective action to DMG by July 11, 2003. <br />A fuel tanker trailer was located on site, but was empty. The operator may only be storing this tanker on site, but is reminded <br />that the permit does not allow for any hazardous materials to be stored on site except within a storage trailer, with adequate <br />spill containment. Additionally, there were 2- 55 gallon drums, a 35 gallon drum, some gas cans and a small (fuel?) tank stored <br />outside the generator trailer for the wash plant. The operator needs to identify the contents of these containers, and, if they <br />contain hazardous materials, either remove these containers from the site, place them in the appropriate containment trailer, or <br />submit a technical revision to the permit to incorporate plans for a spill containment facility for storage of these materials. This <br />corrective action must be completed by July 11, 2003. <br />The reclamation cost estimate was not recalculated as part of this inspection. The current estimate was recalculated less than <br />three years ago and the operation is in compliance with the approved mine plan and reclamation plan. Therefore, the current <br />financial warranty is believed to be adequate to reclaim the site per the approved reclamation plan. <br />The mine identification sign and affected area boundary markers are in place and in compliance with Rule 3.1.12. The Phase I <br />affected area boundary markers will need to be relocated upon approval of Amendment No. 1. <br />There are no large cottonwoods located in the area proposed to be excavated in Amendment No. 1.
