(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1998-084 <br />INSPECTION DATE 02-14-2002 <br />INSPECTOR'S INITIALS SSS <br />This was a routine inspection conducted as part of an on-going monitoring plan for permitted mine sites. This inspection also <br />served to assess current site conditions for use in updating the reclamation cost estimate, ensuring the current financial <br />warranty is adequate to reclaim the site. <br />The mine identification sign was not observed and must be installed per the requirements of Rule 3.1.12 (1) that state: The <br />operator shall post a sign at the entrance to the mine site which shall be clearly visible from the access road and specify the <br />following; a) the name of the operator; b) indicate that a reclamation permit for the operation has been issued by the <br />Colorado Mined Land Reclamation Board, and; c) the permk number. The lack of an identification sign is noted as a problem <br />with the corrective action(s) and correction date specified on the last page of this report. <br />No significant erosional features were noted on the affected areas of the site. <br />The site remains unmined, but vegetation clearing has taken place on the initial pit area. This clearing appears to have taken <br />place over a year ago resulting in a stand of annual weeds across the cleared area. The infestation, if not controlled, will most <br />likely hinder andlor delay successful reclamation of this site and contribute to off-site spreading and establishment of these <br />weeds. A weed control plan developed in consultation wfth, and approved by, the appropriate local weed control authority must <br />be submitted as a technical revision to this permit. Weed infestation is noted as a problem, with the corrective action(s) and <br />correction date specified on the last page of this report. Significant progress toward control of on-site weeds must be <br />demonstrated within three years to avoid potential Board action. <br />Additional site disturbance is limited to two areas where importation and dumping of materials has taken place. In the <br />southeast end of the permit area it appears that two tractor-trailer loads of trash and construction scrap were dumped. This <br />debris needs to be removed and properly disposed of. At the south central site boundary, an estimated 150 dump truck loads <br />of fill material and loads of tom-up asphalt have been dumped. The operator needs to specify the proposed final disposition <br />of these materials. For materials imported for use as permanent fill, the operator will need to submit the notice required by <br />Rule 3.1.5(9). <br />In an effort to ensure the financial warranty remains adequate to reclaim this site per the requirements of the approved <br />reclamation plan, DMG has updated the reclamation cost estimate (copy enclosed). Clearly, when this site is activated and <br />is operated according to the approved mine plan, the operation will be significantly under-bonded. However, with the minimal <br />current disturbance at the site, the current financial warranty remains adequate. However, prior to initiating excavation, <br />Grand Junction Pipe 8 Supply Co. must submit adequate financial warranty to DMG in the amount of $78,696 as <br />shown on the enclosed reclamation cost estimate.
