The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1 <br />2 <br />CORRECTION <br />DATE <br />There was not a mine identification sign posted at the 6/5/98 <br />public entrance to the mine site as required by Rule PB or PV: PB <br />3.1.12(1)(a),(b) & (c). TOPIC(S): SM <br />Corrective actions are to <br />1) Place a mine identification sign at <br /> the entrance to the mine site <br /> (quarry) that contains at a minimum: <br /> the name of the operator and "MLRB <br /> Permit # M-79-206. <br />2) Submit a photograph to our office <br /> which clearly shows that the <br /> identification sign was installed at <br /> the mine entrance. <br />The current financial warranty is inadequate to reclaim <br />the mine if the processing plant is required to be <br />reclaimed as part of this permit. <br />Corrective a ctions are to: <br />1) Submit a letter to the Division that <br /> clearly describes all uses of the <br /> processing facility including any <br /> commercial (please include <br /> documentation) activities. <br />2) Once the Division receives and <br /> reviews this letter, a determination <br /> will be made about whether or not the <br /> plant should be included with Permit <br /> No. M-79-206 and the operator will be <br /> instructed about either: a follow-up <br /> inspection to determine the <br /> appropriate financial warranty or, <br /> how the operator should proceed to <br /> delete the processing plant from <br /> Permit M-79-206 (ie: technical <br /> revision to the permit, partial <br /> liability release, full liability. <br />6/5/98 <br />PB or PV:PB <br />Topic: MP, FN, <br />SF <br />
