(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977.378 <br />INSPECTION DATE 10/14/04 <br />INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred as part of DMG's review of TR-27, addressing the final configuration of <br />Mayflower Tailings Pond 4 and discontinuation of monitoring for phreatic levels, subsidence and for <br />cyanide. The Sunnyside Mine is approved for 159.9 acres affected area. DMG holds $500,000 financial <br />warranty. Operator has commenced final reclamation. <br />The final configuration of Tailings Pond 4 appeared consistent with the descriptions and designs <br />submitted in TR-27. The top of the pond had been graded to provide positive drainage to the west <br />diversion ditch and capped with approximately 2 feet earthen cover, topsoil replaced, seeded and <br />mulched. A small stockpile of topsoil remained for final spreading as needed. On November 17, 2004, <br />DMG approved TR-27. <br />On December 4, 2003, DMG calculated the cost of reclamation at $308,935, which included <br />approximately $35,000 for placement of 2-foot earthen cover and grading to ensure positive drainage to <br />the west diversion ditch. Observations made during the inspection and recorded in this report verify that <br />the operator has completed significant reclamation since the December 2003 calculation.
