(Page 2)
MINE ID # OR PROSPECTING ID #: M-i9~a-nFa
INSPECTION DATE: n4/i ~m~
OBSERVATIONS
INSPECTOR=S INITIALS: TAS
1. Apre-operation inspection was made of the Larimer Pit, as the operator has submitted an application for an
amendment (AM-2) to add an additional 7.74 acres. The total permitted acreage will be 202.74 acres. Present during the
inspection was Steve and Brad Faucher of Loveland Ready Mix Concrete and Rod Harr of Landmark Engineering.
2. All permanent man-made structures within 200 feet of the future excavation area were confirmed on the mine plan
maps. Many of these structures were already discussed during the previous Amendment No! 1 application. Adequacy
concerns pertaining to AM-2 were discussed with Mr. Harr, permitting contact.
3. The inspection revealed that the operator has done an exemplary job of contemporaneous reclamation in the existing
permitted areas of the Larimer Pit. Mr. Panther indicated that the liner in Pond 2 is completed and a representative of the
State Engineer's Office (OSE) has inspected the liner and given a verbal approval that Pond 2 is now adequate for
permanent water storage. A letter from the OSE confirming the suitability of Pond 2 for water storage will be forthcoming
and submitted to the Division when available. Remaining reclamation at Pond 2 consists of spreading topsoil and re-
seeding the banks above the water line. The operator was excavating materials out of Cell 8 at the time of inspection and
was backfilling and grading to finish contours where possible.
4. Digital photographs were taken and are attached to this report.
5. There were no problems noted.
I & E Contact Address
NAME: Steve Faucher
OPERATOR: Loveland Ready Mix Concrete Inc.
STREET: 18111 W 17M Street, PO box 299
CITY/STATE/ZIP: I ovelanri.
