<br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-1711 <br />INSPECTION DATE: 8-18-n1 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS: ~_ <br />Completed a partial inspection of this site as part of a Mined Land Reclamation Board directive to inspect every mine in the <br />state at least once every four years. This operation was last inspected February 6, 1998. <br />This is an SA acre Regular 112 permit located just north of Salida, west of highway 291. The site is located within an area <br />which is zoned industrial. The post mining land use for the permitted area is mostly industrial. <br />The permit was expanded in 1998; from 22 to 84 acres. Mining will occur in phases (approximately 12 acres each). Depth of <br />mining will be restricted by groundwater levels (usually 28-35 feet deep). Mining is currently occurring in phase 1 in the <br />northwest corner of the permit area. Mining will progress from west to east through the various phases. Excavated material <br />has, in the past, been taken to a wash plant in the west central permit area where it is washed and stockpiled before <br />transporting it to batch plants. Currently, excavated material is brought fo the southeast corner of the permit area where it <br />is washed and stockpiled. The operator created an enclosed area (pond footprint) and pumps water from some water source <br />in order to wash the material. The area approved for wash ponds, a wash plant and stockpiling is located west of the east pil <br />area along the Arkansas River. The new wash pond and wash plant area have not been approved. Therefore, the operator <br />will be required to submit a technical revision fo incorporate the new wash pond location and to revise the location of the <br />wash plant and stockpile areas. This will be noted as a problem on page 3 of this report. The T.R., along with an appropriate <br />fee, must be submitted to the Division by October 23, 2001. The T.R. must contain a new site map showing where the new <br />wash pond is located, where the wash plant is located, and where the washed material is being stockpiled. The T.R. must <br />address reclamation issues in the southeast permit area. Since this area has been converted to a washing operation area, <br />when will it be reclaimed? Will the reclamation of this area be revised? Where is the water coming from? Are there any <br />water rights issues? Has the State Engineer's Office made a determination about any associated water rights? Did the pond <br />footprint encounter groundwater? Will the revised activities adversely affect the power poles or power line within this part <br />of the permit? What does the operator plan to do with the old wash pond area? How deep is the new wash pond? What are <br />the pond dimensions? A copy of this inspection report will be sent to the State Engineer for their review and possible water <br />rights determination. <br />A mine ID sign has been placed at the entrance of the permit area as required by Rule 3.1.12. The permit boundaries are <br />adequately delineated. <br />The operator constructed a large noise/visual berm within the 100 foot buffer area on the north side of the permit boundary <br />pursuant to requests made by the citizens who objected to the 1998 amendment application. <br />The junk tires etc. have been removed from the area outside the north permit boundary as requested by objecting citizens. <br />The financial warranty requirement will be recalculated after the Division receives and reviews the above noted technical <br />revision application. <br />Did not observe any other problems at this site. <br />~$" n~~ <br />~ ~ooo ll,7o oooa <br /> <br />I R E Contact Address <br />NAME: Frank Butala <br />OPERATOR: Butala Construction Co. <br />STREET: P.O.
