(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1995-005 <br />INSPECTION DATE 1 /1 07/01 INSPECTOR'S INITIALS GRM <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology's regular monitoring program. The Orchard <br />Mesa Aggregates Pit is located south of Grand Junction, Colorado. The inspector was not accompanied by operator's <br />representative for this inspection. Operator's representatives were contacted but did not show up as scheduled. The <br />inspection occurred at 1005 on November 7°i, 2001. The weather was cloudy and cool. <br />The mine identification sign was not observed and must be installed per the requirements of Rule 3.1.12 (1) that state: The <br />operator shall post a sign at the entrance to the mine site which shall be clearly visible from the access road and specify the <br />following; a) the name of the operator; b) indicate that a reclamation permit for the operation has been issued by the <br />Colorado Mined land Reclamation Board, and; c) the permd number. The lack of an identification sign is noted as a problem <br />with the corrective action(s) and correction date specified on the last page of this report. A sign for permit # M-1990-145 was <br />observed, not one for #M-1995-005. <br />The pit was not in operation at the time of inspection. Several stockpiles of material of various size were noted. A small <br />portable grizzly and conveyor were observed on site. General Mine Plan calls fora 30 feet buffer around power poles <br />transecting the permitted area. The operator appears to be in compliance with this requirement. <br />It appears that fill material has been imported to the sfte for use as structural till. DMG records do not indicate that the notice <br />specified in Rule 3.1.5 (9) has been submitted for this backfill importation and placement. Please submit this notice by the <br />correction date specified in the Problems section of this report. Concrete pipe and slab debris specifically have been brought <br />in to the permitted area. <br />Erosion was noted along the west side of the permit boundary near the imported pipe. Small gullying and rilling were noted <br />in this area. The erosion, although minor, is depositing material outside the permitted boundary. This deposition of materials <br />is Considered off-site damage and failure to control stormwater and is noted as a problem with the corrective action(s) and <br />correction date specified on the last page of this report. <br />Topsoil stockpiles were noted as not having the proper erosion control measures per Rule 3.1.9(1).
