(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1985-003 <br />INSPECTION DATE 04/04/2007 <br />INSPECTOR'S INITIALS DEG <br />This inspection of Permit Number M-1985-003 (Freund Pit) was conducted by Deb E. Gbnima of the DRMS. Mr. Joey Freund <br />was present during this inspection. The pit is located 18 miles northwest of Kiowa, CO, and currently consists of approximately <br />30 acres where approximately 10 acres have been disturbed. <br />This inspector observed some recycled blacktop stockpiled on the mine site (see photo 2). It was explained to this inspector that <br />this material is being stockpiled onsite and will be left onsite at the request of the landowner for continued road maintenance. <br />The Division requires written documentation of this agreement for its records. Therefore, the Operator is required to submit the <br />following information in writing to the Division on or before May 30, 2007 (see Problem No. 1 on page 3): <br />• Written documentation of the intended use of this asphalt; <br />• W ritten documentation of the wishes of the landowner to use this material for road maintenance and that the material be <br />left onsite after reclamation and final surety release; <br />• Written statement committing to not bury the asphalt or in any way use this material as backfill or pond fifh, <br />• A narrative andlor a map that describeslillustrates the approximate location of the asphalt stockpile; <br />• The approximate volume of the asphalt; <br />• Signature of the operator and landowner. <br />Mine entrance and permit boundaries were well-marked. There was one main identifiable material stockpile on the site. The <br />pond on site had well-established vegetation that appeared to be natural rather than seeded. All other activities and completed <br />work at the time of inspection appeared to follow the guidelines and regulations set forth by the state and the mining and <br />reclamation plans for the pit, and no other problems are noted for this site. The site appears stable, and no disturbance was <br />observed outside of the approved permit boundaries. For future reference, this site is correctly positioned in the DRMS online <br />GlS. <br />Responses to this inspection report should be directed to Deb E.
