<br />(Page 2) <br /> <br />MINE ID « OR PROSPECRNG 1D »: M-1 oAf7-~a4 <br />INSPECTION DATE: S-9-f)t IfdSPECTOR'S INITIALS: ~;~ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of [he site on 5-09 01. The main purpose of the inspection <br />was, to collect quarterly water samples and make sure the storm water diversion structures that are with in <br />the jurisdiction of the Division were in proper order. Present during the inspection were Messieurs Phil <br />Barnes, Mark Tidquist and Lawrence Myers and Miss Amei Meketi of CC&V. The inspection began at the <br />underdrain discharge area where three eight ounce samples were collected for WAD cyanide analysis from <br />the solid 24 inch pipe, the south underdrain perforated 24 inch pipe and the 4 inch pipe. Operator collected <br />their own sample using their own collecting bottles. Bridgework was continuing in the area. All the bridge <br />piers appear to be in place. Portions of the horizontal steel beams were in place along the West Side of the <br />bridge. Inspected the energy dissipater below the pond along the West Side it has been expanded as <br />discussed in the previous inspection report. <br />Inspected the phase III construction area. Road relocation and storm water diversion work was continuing, <br />temporary storm water diversion structures in the area were in good repair, no major problem noted. <br />According to Mark, all the work in the area should be completed by the end of the month. Inspected the <br />phase II are pregnant pond and collected the following readings from the high and low volume solution <br />collection transducers. High Volume transducer # 4 was recorded at 9.3, # 5 were at 14.4 and # 6 were <br />14.9 feet of head.
