Walther <br />Eueculive Director <br />Michael 8. Long <br />Division Director <br />Your mine site was inspected on August 16, 2001 by the DMG. Two problems were noted <br />during this inspection. One problem involved the southeast corner permit mazker and the other <br />problem involved the mine ID sign. <br />The southeast comer of the permit boundary was not adequately delineated and you were <br />required to place a marker, or monument, at the southeast corner by the October 23, 2001 <br />corrective action due date. <br />A mine ID sign was not observed at the entrance of the permit area during the August inspection <br />as required by Rule 3.1.12(1). You were required to install a mine ID sign at the gated entrance <br />to the permit area by the October 23, 2001 corrective action due date. <br />The site was re-inspected on November 7, 2001. This inspector found that the southeast corner of <br />the permit boundary was adequately marked and a mine ID sign had been placed at the entrance <br />of the permit azea. However, the permittee's name on the sign was incorrect. You were instructed <br />to correct the sign and notify the DMG that it had been corrected (send a photograph) by <br />December 19, 2001. To date, the DMG has not received any correspondence, or a photograph, <br />from you indicating that the corrective action was completed. <br />You should be aware that if the sign is not corrected, the problem could become a violation. <br />Please correct the mine ID sign and notify the DMG by letter that it has been corrected by May <br />1, 2002. Otherwise the site will be re-inspected and, if the sign has not been corrected, a Reason <br />to Believe letter (that a possible violation exists) will be sent which will require you to appear <br />before the Mined Land Reclamation Board at their June 26-27, 2002 meeting to explain why the <br />problem was never resolved. <br />If you have-any questions, please give me a call at 303-866-4060. <br />
