(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-ta9~-nun <br />INSPECTION DATE: ~-t ~-ns INSPECTOR=S INITIALS <br />OBSERVATIONS <br />This inspection was for the purpose of monitoring the operator's compliance with the permit and regulations. Phil Courtney <br />represented Lafazge West, Inc, and accorrrpanied the inspectors. <br />A sign, at the entrance to the mine, identified the operator but did not have other information required by Rule 3.1. t2. It did not show <br />that a reclamation pemrit had been issued by the Colorado Mined Land Reclamation Boazd and it did not show the permit number. <br />The permit and affected land boundazies were adequately mazked. <br />Mt. Courtney stated that mining has ceased at the Hartman Pit. The mine is being reclaimed. Neazly the entire site has been graded in <br />accordance with the permit approved grading plan. Two small vertically cut banks were observed. One is on the west side of the <br />affected azea directly opposite the entrance road and the other is neaz the northern most edge of the area affected by the operation. <br />These two cut banks must be graded to 3h : 1 v or less, topsoiled and re-vegetated, unless the pemrit is revised to allow them to remain. <br />If such a revision is submitted, it must include a letter from the landowner stating that he wants the two features IeR and not reclaimed, <br />as the pemrit would otherwise require. <br />The northern most part of the approved affected land azea, that part where the gas pipeline is located, had not been mined or otherwise <br />disturbed. <br />Topsoil has been replaced on a portion of the graded affected azea; however, only a small amount of topsoil remains in stockpiles for <br />the remaining topsoil requirement, which is significant. There is not enough topsoil remaining in stockpiles to complete the required <br />reclamation. Lafazge must prepaze a plan to provide an adequate plant growth medium for the areas where there is not enough topsoil <br />This problem is noted on page (3) three. Lafarge must submit a technical Revision to the pemtit by the corrective action due date to <br />resolve the problem <br />There aze some product and/or reject stockpiles that the operator must remove or otherwise dispose of. - <br />The haul road, inside the permit area, must be reclaimed. <br />The bond amount held by the division is $139,285.00.
