<br />III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />This was a complete quarterly inspection of the Munger mine site and office records. The site inspection was <br />conducted on the morning November 18; Loma loadout at approximately 10:45 am, mine site area at <br />approximately 11:45 am. Rodney Head of CAM was present during the inspection of the mine area but not the <br />Loma loadout. Weather was clear and mild. Site conditions were largely unchanged form those noted in the <br />previous inspection report dated 11-2-05, from the 11-1-05 inspection.. <br />Roads <br />At the first (upper) CMP road culverts below the mine bench, further maintenance is still required. This <br />is a 48" culvert. The fabric outlet sleeve associated with the culvert installation was previously torn off <br />and needs to be replaced. Operator hatl inadvertently ordered the wrong sized culvert sleeve, and will <br />need to re-order. <br />The outlet sleeve had been properly replaced on the third road culvert below the mine bench, since the previous <br />inspection. <br />At the fourth CMP road culvert, the inlet was partially obstructed by a large grass clump that needs to be <br />removed. Installation of a series of water bars between the third and fourth CMP road culvert inlets is <br />recommended, to direct road drainage to the inside road ditch. <br />Recent flows had washed out several of the water bars fhat had been recently installed along the two-track road <br />that provides to access to the small topsoil stockpile adjacent to the lower segment of the Munger Canyon haul <br />road. These water bars need to be restored and enlarged, as appropriate. <br />Road drainage ditches and culverts recently installed in association with the recent waste disposal area <br />development and road realignment appeared to have functioned well. However, some maintenance is <br />warranted. <br />Some rock and debris has lodged immediately in front of the inlet to the 36" CMP road culvert located <br />adjacent to the waste disposal area sediment pond, and needs to be cleaned out. <br />The outside road ditch segment (north side of road) between the straw bale pile and the refuse area <br />topsoil stockpile needs to be graded to remove sediment build-up. <br />Hvdrolooic Balance <br />In the refuse area vicinity, the sediment pond appears to have been fully excavated and bermed. The 6" PVC <br />primary spillway tube inlet was approximately 15" above level of the shallow, iced-over pool in the pond. A <br />technical revision to eliminate the existing open channel spillway and install a new CMP spillway was recently <br />approved by the Division, and the approval decision is now final. The approved spillway modifications need <br />to be implemented promptly. <br />There was evidence of additional flow in diversion and collection ditches in the portal area. Diversions and <br />sediment control measures appear to have functioned effectively. There was standing water in the SAE-1 sump <br />area, and sediment had been deposited at the lower end of the sump, to within 18" of the top of the rock check <br />structure. It was evident that there had been flow through the rock check, but it functioned well and remains <br />intact. Rain gage on the mine bench showed 0.8" of rain. Minor maintenance is warranted to re-set silt <br />fence installations below the rock check, in a couple locations. The post which had secured one end of <br />the lower-most fence had rotted out and fallen, and the middle silt fence had been undercut by flow, and <br />nets to be re-trenched and re-set. <br />
