(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2004-025 <br />INSPECTION DATE 4-28-2004 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was apre-operational inspection of the proposed North Fork Pit for which Oldcastle SW Inc., dba United Companies of <br />Mesa County currently has an application under review. <br />The only permanent man-made structures identified within 200 feet of the proposed permit boundary, that were not <br />appropriately addressed in the permit application, are the county shop, the small outhouse located adjacent to the pump house <br />located just east the southeast corner of the site, the culvert under Lane 4005 at the southwest corner of the site, and the small <br />pond located just east of the northeast corner of the site. These structures have been identified in the permit application <br />preliminary adequacy review as needing to be addressed further. <br />The pond on the north end of the site was dry. The pond on the southwest corner of the site contained several feet of water. <br />The Farnsworth spring was flowing a roughly estimated 30-50 gpm. One of the mature cottonwoods, with a swooped trunk, <br />located on the bank of the Farnsworth ditch near the southwest corner of the site is showing signs of stress. While most of the <br />cottonwoods around this ditch are fully leafed out, this tree isjust starting to leaf out. This is noted in case this tree dies during <br />mining, it won't be automatically assumed that the dewatering operations caused the fatality. <br />No river bank instability was noted, other than the sharp bend in the river where large riprap has been placed on the outer bank <br />to stop the side-cutting. The permit addresses modifying and beefing up this armored bank. <br />Whitetop was noted in several locations of the proposed permit area, but most notably in the southeast comer. DMG <br />recommends application of an appropriate herbicide to these noxious weeds as soon as possible and at least annually in an <br />effort to minimize potential infestation of the site, especially after it is disturbed. <br />Vegetation varies from bare dirt in rangeland type areas to pasture in fair condition to thich cover in the willow patches and <br />cottonwood stands.
