(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_ t gz~nn9 <br />INSPECTION DATE: 512£i/ne INSPECTORS INITIALS: A IW <br />OBSERVATIONS <br />This was a normal monitoring inspection conducted by Tony Waldron of DMG along <br />with Steve O'Brian representing Lakewood Brick. The site is located <br />approximately 1-2 miles northwest of the intersection of Plainview Road and <br />Colorado Highway 72. The pit is situated on the east flank of the first foothill <br />looking back east over Rocky Flats. The site is represented as three locations <br />of approximately 2 acres each. The southern most site is the only one that is <br />currently active. <br />The active site has a trench cutting up the hill and to the south. The <br />disturbance was properly delineated with markers and all disturbed areas were <br />contained within these markers according to the maps included in the application. <br />Unfortunately, the maps included with the application are of very poor quality <br />and do not clearly tie in to known points with much confidence. As a result, <br />this is going to be cited as a problem on page three of this report along with <br />corrective actions which will be to submit a technical revision to the <br />application to update the maps to meet the requirements of Rule 6.2.1 and 6.3.1. <br />Since the areas are not contiguous to one another this will also assist in <br />locating the other two areas which were not able to be located with any <br />confidence. However, no other disturbances were observed that would indicate <br />mining of clay is occurring in the near general area. <br />One other area of concern was the run-off coming from the active site. This <br />water reports to the base of the slope prior to discharging off site with little <br />to no treatment. The operator should implement best management practices to <br />treat this run-off by slowing it down and removing some of the sediment. The <br />installation of a small dozer basin should accomplish this task. If there is a <br />storm-water management plan in place for this site it should provide some <br />guidance in this area. If there is not a SWMP in place the Colorado Department <br />of Public Health and Environment should be contacted to see if one is needed for <br />-- - - -- - -- --- <br />this- si`t~- - TFie -fi`nancia7 -warranEy f-or tFiis site is $5, 000.00 and is adequate to <br />reclaim the present disturbance. <br />This concluded the inspection.
