Long <br />Drv¢ion Director <br />Re: Currier Gravel Pit, DMG Permit No. M-80-203, Outstanding Problems Concerning Need <br />for a Permit Area Map and Performance Warranty. <br />Dear Ms. Currier, <br />As you may recall I inspected your grave! pit, named above, on 12/1 ]/97, during which inspection <br />I noted a couple problems. These involved the lack of visible permit boundary markers and lack <br />of an adequate permit area map. Later, I informed you of the lack of a performance warranty in <br />the file, as a problem to address in a timely manner. You were sent a new performance warranty <br />form to use if the original one was not able to be located. <br />As a follow up to the problem of boundary markers, you provided photos of the markers which <br />were placed in 1996, and I deemed that problem to be abated (in my 2/6/98 letter to you). I <br />additionally mentioned in that letter that the need for a map was still an outstanding problem, plus <br />I mailed you a new performance warranty form. <br />The lack of a permit map was an inspection problem to be corrected by 5/31/98. It still has not <br />been received by this office. In fact, item # 1 ] on your 1997-98 annual report, which eve received <br />on 6/17/98, stated that "Acreage is correctly depicted on previously submitted map." There is no <br />map in the file presently, and this problem must be corrected at this time. (If you were able to <br />locate and mark the four corners of the 9.0-acre permit area, there must be some type of survey <br />records or map that was used as a reference, and which may be submitted toward abating this <br />problem. If there was no map or measurement used, the marker locations and acreage may be <br />unreliable.) <br />Concerning the lack of a performance warranty, my 2/6/98 letter to you stated that you must <br />either provide a copy of an existing warranty or complete and submit the new one (the blank form <br />enclosed with that letter). This office still has not received either from you, and this problem must <br />be corrected at this time. The corrective action date for both these problems is set at 2/4/99. <br />Unless 1 hear from you concerning an extension, I will expect your submittals by that date. <br />am not enclosing the 12/1 1/97 inspection report, my 2/6/98 letter to you, or another <br />performance warranty form at this time.
