~~ ~ • <br />INSPECTION REPORT - PAGE 2 <br />FILE N0. M-74-004 <br />DATE: May 2, 1986 <br />OBSERVATIONS OF IMPORTANCE: <br />1.) Inspector met with Mr. Jim Piper at the site. <br />C~ <br />2.) The lower sediment pond was recently cleaned, however, flow can still <br />leave the site along the north side of the sediment pond. <br />3.) Topsoil/growth medium piles are located around the base areas of the <br />mine. Anew pile has been placed between the scale house and the <br />adjacent restaurant. The pile is currently not vegetated. <br />4.) Most check dams are relatively full along the main haul road. <br />5.) The ditch/stream along the north east area is receiving some sediment <br />from the plant area. Material piles have encroached upon the stream. <br />6.) Gullies with associated downhill sedimentation were observed at the <br />following locations. <br />a.) Behind the restaurant; sediment is covering the parking lot road. <br />b.) Behind the northeast gravel piles, leading into the northeast ditch <br />channel. This has been mitigated by placement of a gravel pile uphill <br />of the gully. Should be plugged. <br />c.) Also behind the northeast pile storage area (northeast of small <br />scale). This gully feeds a flat grassy area, however, it should be <br />plugged. <br />d.) A large gully has developed which leads into the northeast: gulch. <br />The gully originates at the upper plant level (quarry bench) and carries <br />down into the gulch. This gully is large and seems to convey much <br />sediment during runoff. The gully should be checked by placing a rock <br />berm at its head (on the plant bench). <br />7.) Care must be taken to avoid placement of any stockpiled materiel over <br />the northeast berm edge and into the northeast gulch. <br />RECOMMENDATIONS: <br />1.) Enlarge the main (access/entry road) sediment pond and berm to trap all <br />flow. <br />2.) Vegetate the new growth medium pile as soon as possible. Mulch may be <br />useful if the seeding season has been missed. <br />3.) Update us on the status of the test mulch plots mentioned in the <br />Division inspection report of June 11, 1985. <br />
