ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19Rn-~aa <br />INSPECTION DATE: I I-'~n-o~ INSPECTORS INITIALS: -BYE <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site with Bruce Humphries, Minerals Program Supervisor <br />and Harry Posey of the Division. Phil Barn of CCBV WAS ALSO PRESNT AT THE TIME OF THE INSPECTION. The <br />Inspection began at the American Eagle look out area, where Phil discussed the project area by pointing at the <br />different activities taking place. Continued the inspection to the East Cresson pit area, it was active at the time <br />of the inspection, no major problem noted. Inspected the main Cresson pit area, Phil explained the activity in <br />the pit, and what they proposed to do in the coming months. Inspected the Phase IV newly deployed ore <br />storage area. Phil explained briefly about the steps CCBV is taking to increase the percolation rate of the ore <br />body. While in the area, the following readings were taken from the Phase IV, high and low volume solution <br />collection transducers. Low volume solution collection transducer # 1 was recorded at 6.8 # 2 was at 2.2. (All <br />readings are in inches of hydrostatic head). High volume collection transducer # 7 was recorded at 13.3 # 8 was <br />at 13.4 and #9 was at 13.8 and 39 was at 13.8 (All readings were in feet of hydrostatic head). <br />Inspected the additional spill prevention area, near the ARD building, Phll explained what was done and why <br />CCBV spent the additional cost to properly line the area even though the area will never be loaded with ore <br />Inspected the Phase II High and Low volume solution collection systems and recorded the following readings <br />from the transducers. <br />Phase II High Volume Solution Collection transducer # 4 was recorded at 18.5 #5 was at 17.6 and # 6 was at <br />17.8. Piezometer was recorded at 28.7 Low volume solution collection transducer # 1 was recorded at .16 and # <br />2 was at .38. (All transducer readings unless noted are in feet of Hydrostatic Head). Inspected the under drain <br />area, both the south and 4 inch pipes were discharging at the time of the inspection, the north under drain was <br />still dry. The discharge from the two pipes was being pumped back onto the pad. Affer the inspection a brief <br />discussion was held regarding the addition of Ammonium Hydroxide to the ore being leached. The Division was <br />supposed to have a meeting with CCBV to discuss, the long-term impacts to the detoxification of the pad. Mr. <br />Barnes requested that since CC&V had not finished the data, from the pad, that the meeting be postponed until <br />such time CC&V decides If the addition of Ammonium Hydroxide w111 be part of the leaching process. At that <br />time, CCBV and DMG will sit down and figure out, what additional measures in terms of ground water <br />protection, by setting numeric standards if applicable and added financial detoxification warranty, to lower the <br />TOTAL NITRATE AND NITRITE FROM THE SOLUTION.
