' • (Page 2) <br />MINE ID # OR PROSPECTING IU # M-83-176 <br />INSPECTION DATE 3/2/2001 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS TAS <br />1. A site inspection was made of the Speer Mining Resource as part of the Division's on-going <br />hydrologic investigation of alluvial sand and gravel operations in the Henderson area. The <br />Division received a number of citizen complaints from surface and groundwater users in the <br />Henderson area in 2000 alleging offsite impacts from active mining operations. Present <br />during the inspection was Tim Buterbaugh. <br />2. The Speer Mining Resource is permitted for a total disturbance of 52.29 acres. The mine <br />plan specifies that a majority of the disturbed area will be dewatered and dry mined. A <br />slurry wall will be installed around the pit area prior to pit excavation. The reclamation <br />plan specifies creation of a 20-acre water storage reservoir to achieve a post-mining land <br />use of developed water resources. The operator has posted a financial warranty of $562,813 <br />in the form of a corporate surety. <br />3. Disturbances noted during the inspection consist of approximately 1 acre of groundwater <br />exposed and stripping of topsoil resources. <br />4. Correspondence from the Office of the State Engineer (OSE) indicates that the Speer Mining <br />Resource needs a temporary substitute supply plan and a well permit. Neither have been <br />issued; however, the operator has filed applications for each and is actively cooperating <br />with the OSE to securing their approval. Please see attached correspondence. <br />5.
