(Page 2) <br />MINE ID # OR PROSPECTING 4D #: M-2003-037 <br />INSPECTION DATE: 9/23/04 INSPECTOR'S INITIALS GRM <br />OBSERVATIONS <br />This inspection was conducted as part of the normal pre-operational review established by the Colorado Division of <br />Minerals and Geology for new or amendments to active permits. Mazk Haldorson, the operator for this permitted <br />site accompanied the inspector. Haldorson Sand and Gravel is amending their permit to add in an additiona165 <br />acres into the permit area and remove 40 acres from the far south end for a net gain 24.27 acres. The total area that <br />will be affected by mining with the addition will increase to 180.27 acres. <br />The proper Public Notice was observed posted at the end of 6200 Road, which is the only entrance to the area. The <br />Notice meets the requirements of Rule 1.6.2(1)(b). <br />A review of the Pre-Mining Map which shows existing structures and utilities appearing as noted with the exception <br />of overhead power lines stretching across proposed Phases B-1 and B-2. The poles are noted so it appears to be an <br />omission during printing of the map. Since a new man is beine prepared, the omission ofthe OHV lines noted needs <br />to be corrected. <br />A further review of the maps shows a small trianQtrlar section in the northeast corner does not match maps approved <br />in the ori~nal permittingprocess. The original maps show the permit corner being up to the right-of way for 6200 <br />Road. No acreage reduction was requested so this azea still remains within the permit boundaries. Since this is part <br />of the original permit and not part of the amendment azea it appears to be an error again in printing. Staff request <br />that the applicant clarify this error and correct the maps. No additional action is necessary. Please be aware that <br />such omissions that include amended azeas could result in re-notifications of adjacent landowners and decision time <br />clocks being started anew. <br />A major discussion was the proposed mining plans and text describing those plans. Mining_Plan text will need to <br />clarify how materials from the different phases will exit the permitted site through Phase 1 access while maintaining <br />concurrent reclamation. Permanent haul roads should be denoted on the map with a clarification that they will <br />remain afterwazds for farming access. Temporary roads should be noted for reclamation after mining is completed <br />with rough descriptions of approximate location and size. Current text cuts off Phase 5 through 13 if reclamation is <br />done as described. <br />There aze some structures that the operator is unsure exactly when they will be removed. Staff understands that <br />mining timetables and conditions may shift. The primary concern is that most of these structures are occupied <br />although owned by the operator. The Division's concern is that mining plans do not spell out how these occupied <br />structures will be safeguarded in the interim. Staff request a plan to ensure that mining does not effect these <br />structures while occupied. <br />No other issues were noted at this time. <br />
