<br />INSPECTION REPORT - PAGE 2 <br />FILE N0. M-82-090 <br />DATE: dune 18, 1986 <br />OBSERVATIONS AND RECOMMENDATIONS: <br />1. Mr. Charles Ponchak accompanied the inspectors on the inspection. <br />2. Tailings NE of the office building were being removed for the <br />construction of a sediment pond for 001 and 004 decants. <br />3. The upper and lower active tailings ponds had solutions ponded on their <br />surfaces. <br />4. Water was observed in the seep water impoundment at the toe of the lower <br />active tailings pond. <br />5. A "seep hole" was observed on the SE toe of upper active tailings pond. <br />Mr. Ponchak related that the hole "blows out" each year, later filled by <br />sloughing tails. Please monitor this location and note any enlargement , <br />sloughing, or possible signs of slope instability/failure. <br />6. Canyon Creek is eroding the toe of the tailings pile. Please discuss <br />the possibility of riprapping the toe of the slope to minimize sedimentation <br />of Canyon Creek. <br />7. The topsoil stockpile was not seeded. Mr. Ponchak stated that <br />re vegetation will not commence until topsoil stockpiling is completed, which <br />is anticipated for Fall, 1986. <br />8. Please discuss the implementation of revegetation test plots on tailings <br />material. <br />9. Small patches of vegetation were observed at the toe of the upper active <br />tailings pond, including bluegrass, wheatgrass and horsetail. <br />10.
