III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />This was a partial inspection conducted by Tom Kaldenbach of the Colorado Division of Minerals and <br />Geology. The site was covered with 8 inches of snow. <br />AVAILABILITY OF RECORDS <br />The following documents still need to be inserted in the operator's file that is kept at the Routt County <br />Clerk and Recorder's office. <br />An insurance certificate that contains: a) the wording "will endeavor to" struck, b) the <br />insured's current address, and c) an indication of the type of insurance. <br />2. 3 years of DMGIOSM inspection reports and enforcement actions, <br />3. Midterm Review documents, <br />4. The most recent NPDES Permit, <br />5. Two NPDES Discharge Monitoring Reports (for 7/1/97-8/31/97 and 9/1/97-10/31/97). <br />HYDROLOGIC BALANCE <br />Sedimentation Pond <br />The pond and all ditches were covered with snow and contained no water. <br />SLIDES AND DAMAGE <br />Office SAE <br />The backfilled slope appeared stable. <br />The approximate 2 cubic yards of rock and dirt that were left at the toe of the channel when the gabion <br />baskets were installed should be removed next field season unless there is a use for the material on- <br />site. <br />Page 3 <br />
