(Page 2) <br />MINE IC « OR PROSPECTING ID ~: M-' g•~n-~-+~ <br />INSPECTION DATE: F-~'~-nn INSPECTORS INITIALS: ~IC_ <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the phase III area on 6-23-00. <br />Present during the inspection was, Mr. Scott Lewis of CC&V. The inspection began around the Phase III <br />construction area. Liner deployment in the area has been completed. Drain cover was being placed in the <br />lined area at the time of the inspection. The lined area was walked and inspected, no major problem noted, <br />with the exception of a small bubbled up area that was noted near panel #39. Mr. Bob Nickels of Golder <br />who is co;iducting the QA/QC was notified of the minor problem. Heat::~ared the Division tha: the area will <br />be taken care of during drain cover placement and that the drain cover will be placed at the same <br />temperature as the liner deployment Temperature to remove the bubble prior to placing drain cover. CC &V <br />requested to place spent ore at the thalweg of the Phase III area prior to submitting the certification and <br />obtaining approval from the Division. Since each part of the construction process is supervised by Golder <br />QA/QC personnel and Golder is the design, QA/QC and certifying engineering firm and based on past similar <br />approvals by the Division, the request was granted during the inspection. CC&V is aware that if for any <br />reason, the area can not be certified they assume all the risk. <br />During the replacement of the low volume solution collection pump for phase I, a minor spill out side the <br />protected area had been reported to the Division on Wednesday the 21~. Since all the required remedial <br />actions had been taken by CC&V, which included removal and placement of all contaminated soil outside of <br />the protected area on to the leach pad and detoxi;;ca:ion of the contaminated area and collections of <br />samples for analyses, the area was inspecte ~ on Friday the 23rd. None of the spill reached the waters of the <br />United States, it was all contained within approximately 100 feet from the spill, with in the permit area. <br />The area of the spill was walked and inspected, no major problem noted. Mr. Lewis stated that is was <br />operators error that caused the spill. Prior to replacing the pumps, the operator was supposed to shut off <br />:he val°ae, which he failed to do that • . ~ Ce':rf:d t112 spill OUtSide the protected area. Mr. Lewis assured <br />'t`;a Ji:~ision, in the future, when such tasks are performed, t;~e pere~n or persons who will be doing such an <br />operation will be properly trained.
