• (Page 2) • <br />MINE ID # OR PROSPECTING ID #: M-i 9Rn-~aa <br />INSPECTION DATE: R-»-m INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site while in the area, at the request of the operator. <br />The main purpose of the site visit was to discuss the minor changes the operator wanted to implement in <br />the Phase IV, area leach pad foundation preparation, grading plan. Present during the office meeting to <br />discuss the changes were, Miss Ann Baldridge, Mr. Phil Barnes and Mr. Mark Tidquist from CC&V. They <br />explained that the changes were necessary because CC&V found out a better way of doing cut and fill, <br />during grading the foundation for the Phase IV area. The minor changes included widening the haul road <br />width and some miner changes to maximum slopes. These kinds of changes are allowed under the MLRD <br />BOARD approved amendment # 8. <br />After the meeting, with Phil Barnes present, inspected the main Cresson pit area surtace water diversion <br />structures, they appeared to be in good repair. Iron Clad Globe Hill area was inspected, no major problem <br />noted. Constructions of the emulsion and track shop buildings were continuing, no major problem noted. <br />Continued the inspection to the Phase III area, work on the highway 67 relocation and storm water <br />diversion was continuing, no major problem noted. All three lower Arqua Gulch ponds were holding water <br />and discharging at the time of the inspection. Inspected the Phase II area and collected the following <br />readings from the high and low volume transducers. High volume transducer # 4 was recorded at 11.4 feet <br />of head, # 5 was recorded at 13.7 feet of head and # 6 was recorded at 15.9 feet of head. Low volume <br />transducer # 1 was recorded at .07 feet of head and # 2 was recorded at .19 feet of head. All were in <br />compliance with the approved permit condition. <br />-Inspected the external pond, as reported in the earlier inspection report of 8-16-01, storm water from the <br />lower Arequa pond was being pumped into this pond. The pond appeared over half full. Even though the <br />certification report for this pond has not been approved by DMG, the fact that both the design and qA/QC <br />engineer is Golder and associates and have been on site to verify each aspect of the pond was built as <br />approved, satisfied the Division to allow the temporary placement of storm water in the pond. Once the <br />solution recovery system is in place, CC&V will submit the certification report for approval.
