ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_tagn_~dd <br />INSPECTION DATE: t~/17/03 INSPECTOROS INITIALS: <br />OBSERVATIONS <br />The Division conducted a monitoring Inspection of the site on 12/ 17/ 03. Present during the inspection were <br />Mark Tldquist and Tlm Comer of CC& V. All backfilling and grading in the soil burrow area is completed. All the <br />mined out soil from the area is now stock piled in the Arequa waste storage area. CC&V plans to amend the <br />soil with bentonite prior to placing it in the Phase IV C leach pad construction area, in Sprig 2004.Continued the <br />inspection to the Phase IV PREGNANT SOLUTION COLLECTION AREA. The following readings were recorded from <br />the High and Low volume solution collection transducers. Low Volume Solution Collection transducer # 1 was <br />recorded at 7.2 and # 2 was at 6.9 inches of head. As requested by DMG, CC&V had placed a log book that <br />showed all the readings recorded every 4 hours, by each shift operators. High volume solution collection <br />transducer # 7 was recorded at 15.5 # 8 was recorded at 15.6 #9 was recorded at 15.7 and # 10 was at 16.1 feet <br />of hydrostatic head. <br />Inspected the Phase I and II pregnant solution collection areas and recorded the following readings from the <br />Phase I and II High and Low Volume solution collection transducers. <br />Phase 1 High Volume Solution Collection transducer # 1 was recorded at 32.1 # 2 was at 34,5 and # 3 was at 34.0. <br />Low volume transducer for Phase I was being replaced at the time of the inspection. <br />Phase II High Volume Solution Collection transducer # 4 was recorded at 22.3 # 5 was at 22.0 and # 6 was at <br />22.3.Low volume transducer # 1 was recorded at .16 and # 2 was at .31 (all readings are In feet of hydrostatic <br />head) <br />Inspected the under drain area. Both the south and 4 loch pipes were discharging a trickle.
