(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-ioan_~ae <br />INSPECTION DATE: ~-~~-n3 <br />INSPECTORS INITIALS: -9fL <br />The Division conducted a monitoring and construction progress inspection of the leach pad facility on 6-30-03. <br />Present during the inspection were, Harry Posey, from the Division Mark Tidquist and Phil Barnes from CC&V. The <br />Physical inspection began around the Phase IV leach pad expansion areas. Both Phil and Mark explained the <br />construction sequence to Harry Posey. The riser pipes for the pregnant solution collection pipes had been <br />extended to accommodate the first ore lift. Operator plans to apply solution in the area, as soon as the <br />temporary pipes and transducer set-ups are in place. The certification report for the PSSA AREA was submitted <br />to the Division on 6-2b-03. The DIVISION is in the process of reviewing the report, however, like in the past, <br />operator was granted to apply solution, before certification approval. The Division was provided weekly <br />construction reports, In addition to physically being present on site, once. a week .All minor changes to the <br />Board, approved plan under Amendment # 8, ware granted by the Division. As in the past, the operator (CC&V) <br />is assuming all the risks, by applying solution, before receiving the written approval. The Division was provided a <br />draft certification report a week before the actual submittal and NO FATAL FLAWS WERE DISCOVERED. <br />The first ore lift was being deployed on the PSSA area, along the west side. Clay liner, geomembrane and drain <br />cover were being deployed at different parts of the leach pad facility expansion areas at the time of the <br />inspection, NO MAJOR PROBLEM NOTED. <br />Inspected the Phase I and II pregnant solution collection areas and recorded the following readings from the <br />High and Low volume solution collection transducers. <br />Phase I high Volume solution collection Transducer # 1 was Fecorded at 24.0 # 2 was at 22.0 and # 3 was of 21.0 <br />Low volume solution collection transducer # 1 was recorded at .14 and # 2 was at .33 <br />Piezometer for the area was at 34 feet <br />Phase II high volume solution collection transducer # 4 was recorded at 26.1 #5 was at 20.4 and # 5 was at 28.6 <br />Low volume solution collection transducer # 1 was recorded at .10 and # 2 was at .61 <br />Piezometer for the area was at 28.6 fast. (ALL TRANSDUCER READINGS ARE IN FEET OF HYDROSTATIC HEAD) <br />Inspected the external pond, it was holding some water, while in the area, collected the following readings from <br />the low volume solution collection Transducers. Transducer # 1 was at 9.6 and # 2 was at 11.40 inches of <br />hydrostatic head. Inspected fhe under drain area, both the South under drain and the 4 inch pipes were <br />discharging, the discharge was being pumped back onto the leach pad. Both the upper and lower ponds were <br />holding some water from the recent rainfall the area received. All surface water diversion structures in the area <br />were in good repair. <br />Continued the inspection to the American Eagle Overlook area, where Phil briefly pointed out to Harry, the <br />different parts of the permit area. Continued the inspection to the East Cresson Area, it was active, no major <br />problem noted.
