ORIGINAL-PUBLIC FILE <br />(Page 2) <br />MItJE ID # OR PROSPECTING ID #: ~~-19ap=add <br />INSPECTION DATE: Z(OR%sd INSPECTOROS INITIALS: ~- <br />OBSERVATIONS <br />Th.e Division conducted a weekly construction progress of the Phase IVC <br />leach and area and a monitoring inspection of the site on 7/08/04. <br />Inspected the Phase IVC leach pad area. According to Mark, the area had <br />been receiving a lot of rain every afternoon, which made the clay and <br />ge~omembrane deployment difficult, and only a small amount had been <br />deployed, since the last inspection. The deployed liner appeared to meet <br />tYle required specifications. Drain cover was being deployed along the west <br />facing slopes at the time of the inspection. The weekly construction report <br />from last week reported an incident of liner tear during the deployment <br />process, due to operator error. The repair was properly repaired and tested <br />in accordance with the approved specifications. Golder QA/QC in addition to <br />Ames workers were present over looking the deployment. Ron Roberts, manger <br />of° the construction stated that he had implemented additional measures of <br />two people watching the drain cover push, so the same error will not be <br />repeated. <br />Inspected the phase I, II, III AND IV leach pad pregnant areas and recorded <br />tYie following readings from the High and Low volume solution collection <br />transducers. <br />Phase I High Volume solution collection transducer # 1 was recorded at 18.1 <br />#:? was at 21.0 and # 3 was at 20.6. Low volume transducer # 1 was at .15 <br />and # 2 was at .52. Piezometer for the pond was at 34 feet. <br />Phase II and ITI High Volume solution collection transducer # 4 was <br />rc>_corded at 37.3 # 5 was recorded at 39.3 and # 6 was recorded at 39.0. Low <br />volume transducer # 1 was at .11 and # 2 was at .40. Piezometer for the <br />pond was at 39.6 feet. <br />Plxase IV High Volume solution collection transducer #7 was recorded at 22.9 <br /># 8 was at 22.7 # 9 was at 22.9 and # 10 was at 23.2. Low volume transducer <br /># 1 was at 10.7 and # 2 was at 9.9 inches of hydrostatic head. (All other <br />readings are in feet of hydrostatic head) <br />Inspected the external pond, it was holding some water (rain). While in the <br />a:re recorded the following readings from the Low Volume collection <br />transducers. Transducer # 1 was at 13.4 and # 2 was at 10.6. <br />Il.~spected the under- drain area, all the ponds with the exception of the <br />upper most pond, were dry. Both the south and 4-inch pipes were discharging <br />at. the time of the inspection. <br />
