(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1981-158 <br />INSPECTION DATE 7/12106 INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred in response to a request from Nathan Barton, with Wasteline, Inc., submitted on <br />behalf of the Operator, Forest Lakes Metro District, for an on-site meeting. The Forest Lakes Pit #1 is <br />approved for 6.34 acres affected area for conventional extraction of materials from the Pine River terrace <br />and on-site processing. Affected lands will be reclaimed to support recreation post-mining land use. <br />DRMS holds $11,688 financial warranty. The permit status is intermittently active. Six photos and a <br />reclamation cost estimation, totaling $10,141, accompany this report. <br />The permit area is located within a 40-acre parcel owned by the Forest Lakes Metro District. The parcel <br />contains the district's shop and office building, fire station, solid waste transferstation, fresh waterwells, <br />wastewater treatment plant, materials storage and equipment parking areas. Boundaries between the <br />various activity areas overlap. During the inspection, contractors were working at the waste water <br />treatment plant adjacent to the DRMS permit area. The Operator discussed an immediate need to <br />construct several dikes across the large wastewater "polishing" pond to enhance performance of the <br />waste treatment plant. The Operator wanted to excavate material at the proposed wastewater treatment <br />plant Cell 3 area, to provide construction materials for the dikes and inquired whether a reclamation <br />permit would be required for such an excavation. DRMS replied that the activity appeared to satisfy the <br />third exemption of the MLRB's Policy dated April 29, 2004, but that the Operator should reply to the <br />Division's "Need For State Reclamation Permit" inquiry for a written response from DRMS. <br />Mining activities were dormant during the time of this inspection. Mine identification sign and permit <br />boundary markers were posted. Mining activities were confined to approved areas and appeared <br />compliant with permit conditions. <br />DRMS has reviewed the current cost of reclamation at $10,141. Therefore, the existing $11,688 financial <br />warranty appears sufficient to ensure reclamation.
