The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />AND <br />CORRECTION <br />OS / 12 /06 <br />PB or PV: _P_B._ <br />TOPICS}: _Li6_ <br />Corrective action: The operator must submit on or before May 12, 2006 a stabilization plan, in the <br />form of a modification to the permit as determined by the Division, which predicts the hydraulic <br />conditions of the Colorado River in and adjacent to the affected lands during a 100-year flood event <br />and includes adequate safeguards to minimize the potential for excessive erosion, channel <br />degradation, and/or re-direction of the existing main channel of the Colorado River during final <br />reclamation of the disturbed lands. (Of particular concern are the water intake structure owned and <br />nTneratetl the Cit rnf RiFlP~ anri the fnlnrarln Highww> 14 hri nd c~mT rtc in the r'i~ of RiFlPI_ <br />2. There was no evidence of an outlet spillway in the West Cell excavation, as required by the <br />approved mine plan. <br />OS / 12 /06 <br />PB or PY: ~ <br />TOPIC(S): 1~1L <br />Corrective Actions: Submit verification on or before May 12, 2006 that an outlet spillway has been <br />installed in each cell of excavation, in accordance with the approved designs, as specified in Technical <br />Revision No. 1. <br />Review of the permit file indicates that inlet & outlet <br />spillway designs for each reclaimed cell and copy of the <br />approved 404 Permit from the US Army Corps of Engineers <br />has not been submitted and incorporated into the mining <br />and reclamation permit, as previously committed to by <br />the operator. <br />05/12/06 <br />PB or PV:PB <br />TOPIC (S) MP <br />Corrective Actions: Submit on or before May 12, 2006 a <br />outlet spillway designs for each <br />rec aimed cell and copy of the approved 404 Permit from <br />the US Army Corps of Engineers. <br />
