Zinc was the only metal detected in the analyses, and it was present at low <br />levels (.02 mg/L and .04 mg/L). COM, Inc. has discontinued use of the land application <br />system for 1996, but if the system is reactivated, the following detection limits need to be <br />used for sample analysis: <br />Cu .005 mg/L; Pb .005 mg/L; Se .O1 mg/L; Aq .0002 mg/L <br />Problem no. 5 listed in the 6/17/96 DMG inspection report noted the poor condition of the <br />Hazel-A ad it portal hatch. The Hazel-A ad it Portal hatch has now been replaced with a <br />locking steel door. This ie a substantial upgrade to the previous configuration. In a <br />telephone message left on 8/6/96, John Miner stated that a small gap at the top of the new <br />Hazel-A door frame would be sealed ae soon as a welder ie mobilized to the site. <br />Problem nos. 6, 7, and 6 listed in the 6/17/96 DMG inspection report were related to the <br />condition of the Hazel-A water management eyatem. These problems had been adequately <br />corrected at the time of this inspection. The log book stored in the Hazel-A adit indicates <br />that daily inspections are being conducted. water storage behind the bulkhead was 3/4 to 7/8 <br />full. Paul Jones stated that COM, Inc. ie allowing the water storage area behind the Hazel-A <br />bulkhead to fill to relatively high levels because this is a source of water for ongoing <br />clean-up of the mill building. If the Hazel-A is going to be used to store water, then daily <br />inspection of the water management system will continue to be required. If the Hazel-A is <br />pumped down, then bi-weekly inspections would probably be sufficient at this time. <br />In a letter from DMG dated 2/5/96, coM, Inc. was directed to apply for a well permit for the <br />Hazel-A ad it. To date, an application for a well permit has not been filed. COM, Inc. must <br />document to DMG that a well permit application has been filed no later than 9/1/96. <br />It is DMG'e understanding that the Gold Hill Mill was sold by COM, Inc. to Franklin <br />Consolidated Mining Company, however, the permittee for the reclamation permit continues to <br />be COM, Inc. If COM, Inc. is going to continue as the permittee, evidence of an agreement <br />with Franklin giving COM, Inc. management control and legal right to enter needs to be <br />submitted. Otherwise, application for succession of operator from COM, inc. to Franklin <br />needs to be filed. This situation needs to be resolved by 10/1/96. <br />It was noted during this inspection that the middle decant riser in the tailing pond was not <br />visible. This decant riser needs to be located and reestablished by 10/1/96. <br />The tailing pond area had been recently surveyed and staked to delineate the area that would <br />be affected by installation of the 10 foot tailing dam raise that ie approved in the Gold <br />Hill Mill reclamation permit. The following permit requirements are pertinent to the <br />operator if work on the dam raise is being considered: <br />1. The operator ie committed to providing notification to DMG prior to <br />initiating construction of the dam raise. The notification to the DMG will allow time for <br />recalculation of the amount of financial warranty to be required. Notification will also <br />allow DMG to specify an inspection schedule with construction milestones during installation <br />of the tailing dam raise and liner installation. <br />2. The operator may not disturb any land within the boundaries of the Golden <br />Crown claim until adequate documentation of legal right to enter this claim has been provided <br />to the DMG. <br />3. Prior to using any material from the borrow area above the existing pond <br />to construct the dam raise, the operator must put up a stockpile of well graded rock fill <br />that contains sufficient material to install a 2 foot thick cover over the expanded tailing <br />pond surface. The permitted location for this stockpile is depicted on the maps that are <br />part of the reclamation permit file. <br />4. All materials specifications and construction quality assurance <br />requirements that are included in the approved plan must be complied with during construction <br />of the dam raise. The operator may not commence milling operations at the Gold Hill Mill <br />until construction of the expanded tailing pond has been certified by a registered <br />professional engineer and the certification has been accepted by the DMG. <br />
