(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1997-084 <br />INSPECTION DATE 4/9/04 INSPECTOR'S INITIALS_WHE <br />OBSERVATIONS <br />This inspection occurred as part of DMG's routine monitoring of permitted sites. DMG attempted to <br />contact the permit holder prior to the inspection but was not successful. The Memphis Site is approved <br />for 2.44 acres affected area. Affected lands will be reclaimed to support industrial/commercial post- <br />mining land use. DMG holds $1,053 financial warranty. Operations appeared dormant. No activities <br />were observed, no individuals were contacted during the time of this inspection. <br />Mine identification sign and permit boundary markers were posted. The approved mine and reclamation <br />plan indicates that excavation activities will remove an alluvial fan from the base of vertical cliffs located <br />along the east side of the canyon and east side of Hwy 550. Excavation activities appeared to have <br />begun minimally. Several small cuts into the toe of the slope had occurred. However, excavated <br />material appears to have stayed on site and utilized to extend the roadside shoulder of Hwy 550. <br />Evidence of material being exported from the site was not observed. <br />DMG has reviewed the current cost of reclamation totaling $744. Therefore, the existing $1,053 financial <br />warranty appears sufficient to ensure reclamation. Copy of DMG's April 22, 2004 reclamation cost <br />estimation is enclosed. The reclamation cost estimation covers only the existing disturbance and will <br />increase as the disturbance increases.
