<br />(Page 21 <br />MINE ID # OR PROSPECTING ID # M-77-306 <br />INSPECTION DATE 3/26/97 <br />INSPECTOR'S INITIALS WHE <br />This complete inspection was conducted by Wally Erickson from the DMG Durango office, 484 Turner Drive, Building F, Suite 101, <br />Durango, CO 81301, phone 19701 247-5193. Glen Williams and Robert Schwind, Cotter Corp Nucla office, accompanied the DMG <br />inspector throughout the inspection. The CJD-9 Mine is located in Bull Canyon of Monogram Mesa, approximately 20 miles NW <br />of Naturita, in portions of Section 30, T46N, R77W, NMPM, Montrose County. Permit was issued on 9/30179 for 11.1 acres <br />affected area. DMG holds financial warranty in the amount of S 15,700. Operation was dormant during the time of this inspection. <br />Records -Original permit application was approved on 12/15/77,for 8.25 acres affected area; however, permit was not issued until <br />9/30/97. Subsequent to permit approval and prior to permit issuance, DMG approved two Technical Revisions ITRI. addressing <br />relocated vent holes and evaporation ponds, and one Amendment, increasing affected area to 11.1 acres. On 8127/80, operations <br />went into Temporary Cessation ITCI, due to depressed uranium/vanadium markets. On 8/22/85, DMG approved 5-year extension <br />of TC status. On 8/31 /90, DMG approved TR-01, revising mine status from TC to intermittently active. On 6/6/94, DMG approved <br />TR-02, modifying configuration of affected area boundary to allow additional water treatment ponds. DMG records do not indicate <br />additional modifications to the permit application. <br />Stormwater Management Plan was submitted to WOCD on 9/30/93 for Stormwater Discharge Permit No. COR-040028. On <br />12/30/93, CDPS Permit No. CO-003651, was issued by WOCD of the CDPHE, for discharge from water treatment plant. <br />Signs & Markers -Mine identification sign and affected area perimeter markers were observed. Mine entrance road was recently <br />graded and well maintained. Road drainage is treated for sediment by silt fence prior to leaving permit area. <br />Portal Area -Public access to mine portal is controlled by locked metallic doors. New mining timbers were stockpiled adjacent to <br />portal. Shop building, waste rock stockpile and access roads appeared to be located within affected area. Culvert, 6' diameter and <br />approximately 70'long, under waste rock access road, appeared to be clear of sediment and debris, no discharge observed. <br />Operator stated water treatment ponds, located on top of Monogram Mesa, may be relocated to portal area, through future Technical <br />Revision. <br />Water Treatmertt Facility -Series of two water treatment ponds were located on top of Monogram Mesa. Mine water is pumped <br />to the treatment facility, approximately 600' above the underground workings. Ponds were surrounded by 3.5' high woven wire <br />and barbed wire fence. Mine water is treated with barium chloride prior to discharge. Second pond was discharging approximately <br />1 gpm, discharge appeared clear, no sample taken by DMG inspector. Discharge is piped to mesa rim and released into an un-named <br />tributary of Bull Canyon. Water treatment facility appeared to be well maintained. <br />Verrt Hdes -Fan at vent hole #2 was not operating, fan was covered by tarp. Fan housing and concrete pad were intact. Operator <br />stated vent hole was 7' diameter, lined with steel casing and approximately 650' deep.
