(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1996-031 <br />INSPECTION DATE 6-7-2002 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was a partial inspection conducted at the operator's request to evaluate recent grading and slope reduction that has been <br />performed to reduce the reclamation liability for the site. <br />All highwalls on the North, east and eastern two-thirds of the south highwall have been reduced to slope grades of 3H:1 V or <br />less. Topsoil from the stockpiles has been replaced on the east third of the south pit slope and piles of topsoil have been <br />placed by loader on the remaining pit slopes that have been graded to final slopes. A bulldozer was spreading the topsoil on <br />these slopes during this inspection. The pit floor is being left as a graveled pad for stockpiling and facilities area for future <br />mining. <br />The previous inspection report for of this site noted that topsoil piles were lacking stabilizing vegetative cover. After regrading <br />and topsoil placement on the pit slopes is completed, gypsum at a rate of 1 ton per acre will need to be incorporated into the <br />topsoil on the pit slopes. Seeding with the white clover cover crop and mulching of the regraded pit slopes and remaining <br />topsoil piles will need to take place. This seeding will need to be completed between October 15, 2002 and November 15, <br />2002. Documentation of the gypsum application and seeding must be submitted to DMG prior to November 29, 2002. This <br />documentation must include seed tags, receipts or scale tickets for the gypsum and photographs of the seed and gypsum <br />applications. This information will resolve the currently unresolved problem noted in the previous inspection report. The <br />correction date for that problem is now set for November 29, 2002. <br />The reclamation cost estimate update included with the previous inspection report has been revised to account for the pit slope <br />reduction and topsoil placement observed during this inspection revised copy enclosed.
