• (Page 21 <br />MINE ID k OR PROSPECTING ID tf <br />INSPECTION DATE 3126/97 <br />M-77-307 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS WHE <br />This partial inspection was conducted by Wally Erickson from the DMG Durango office, 484 Turner <br />Drive, Building F, Suite 101, Durango, CO 81301, phone (970) 247-5193. Glen Williams and <br />Robert Schwind, Cotter Corporation Nucla office, accompanied the DMG inspector throughout the <br />inspection. The C-CM-25 Mine is located approximately 2 miles SW of Uravan, adjacent to the <br />Umetco UMTRA tailings disposal project. Portal, shop building, compressor building and waste rock <br />stockpile are located in portions of the NW 1/4 of the NW 1/4 of Section 4, T47N, R17W, NMPM. <br />Vent holes and access roads are located 'in portions of the NE 1/4 of Section 5, T47N, R17W, <br />NMPM. Additional access roads are located in portions of the SW 1 /4 of the SW 1 /4 of Section <br />31, T48N, R17W, NMPM, Montrose County. DMG holds financial warranty in the amount of <br />52,500. Operation was dormant during the time of this inspection. <br />Records -Original 110 application was submitted on 9/30/77 for 5.85 acres affected area. On <br />1 1 /21 /77, operator submitted response to DMG adequacy concerns. Operator 1 1 /21 /77 submittal, <br />dated 1 1 /18/77, requested increase in affected area to 7.34 acres. Permit was issued on 4/30/79, <br />permit document states affected area as being 5.85 acres. DMG database records indicate 5.85 <br />acres affected area. DMG is reviewing records to clarify confusion of permitted acreage, and is <br />requesting assistance from the operator regarding affected area acreage. <br />Uranium and vanadium market conditions in 1980 forced operations into Temporary Cessation <br />status. On 8/22/85, DMG approved 5-year extension of TC status. On 8/31 /90, DMG approved <br />Technical Revision #1 (TR-01-, revising mine status from TC to intermittently active. DMG records <br />do not indicate additional modifications to the permit. <br />Signs & Markers -Mine identification sign and affected area perimeter markers were observed at <br />the portal/shop permit area. Mine entrance road passes through areas affected by UMTRA project, <br />access may be temporarily blocked by UMTRA activities. Access road within permit area was well <br />maintained. Permit areas located in Sections 5 and 31 were not inspected. <br />Facilities -Public access to mine portal is controlled by locked metallic doors.
