(Page 2) MINE ID # OR PROSPECTING ID #: M-ies~~t-ins INSPECTION DATE: ~->>-o~ INSPECTORS INITIALS: aK OBSERVATIONS The Division conducted apre-operational inspection of the site on 6-11-03. Present during the inspection was Mr Carl Randel of Cheyenne County Road and Bridge P.O.BOX 567, CHEYENNE WELLS CO. The site used to be an old CDOT pit that was succeeded by CHEYENNE COUNTY IN 1983. The reason for the pre-operational Inspection was the operator's submittal of an Amendment to Increase the permit area from the present 35 acres to 65 acres. Most of the previously permitted acreage has been mined out. According to what was observed during the pre operational inspection, the site has been inactive for the past few years. The entire amended area was driven and Inspected, Mr. Randel pointed out the County's future mining plan. Permit sign and boundary markers were properly posted. However, there was no permit AMENDMENT FILING NOTICE POSTED. The site was as described in the Amendment application. Mr Randel was notified of the need to post the Notice on the gate or the fence. He requested that I contact Ms CROWELL, over the phone and inform her what needs to be done to be in compliance. Affer the inspection was completed, contacted Ms Crowell, County Administrator and informed her to place the application filing notice in accordance with the Construction Materials Rules and Regulation and provide proof in the form of a Photograph or a letter confirming that it had been done as required by the Rule. She was also asked to provide proof of publication. She agreed to do so and provide the required proof as soon as possible.
