(Page 2) <br />MINE ID # OR PROSPECTING ID #: _ ;,n-79w-3~e <br />INSPECTION DATE: 5/?Sm5 INSPECTORS INITIALS: ~!? <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology four-year monitoring <br />program. The previous inspection was conducted on January 5, 2002, and therefore the site was not <br />due for a monitoring inspection. An inspection was being conducted, however, at an adjacent site. In <br />that access to this site is somewhat rigorous, a current inspection was conducted. The operator was not <br />notified of the inspection, and therefore was not present. Present at the inspection were Kate Pickford <br />and Harry Posey, DMG. Weather at the time of the inspection was partly cloudy, windy, and 65 <br />degrees. <br />2. Access to the site is gained off of County Road 9. A sign is posted at the entrance to the site, which is <br />located at least one mile west of County Road 9. Boundary markers were observed at various locations <br />around the site. The maximum disturbed area for the permit is 18 acres, and it appears that the <br />disturbance at the site has been confined to within this guideline. The current disturbance at the site <br />consists of exposed sandstone and clay deposits. Activity at the site appears to be within the guidelines <br />of the current permit. <br />3.
