(Page 2)

MINE ID # OR PROSPECTING ID #: M-1990-138
INSPECTION DATE: 11/15/01 INSPECTOR'S INITIALS: _~
OBSERVATIONS
This was a normal monitoring inspection conducted by Tony Waldron of DMG along with Kenneth Klco representing Fremont
County. The site is located approximately 30 miles north of Cotopaxi, Colorado in what might be described as a high inter-
mountain park setting.
There was an identification sign posted at the mine entrance and the permit boundaries were clearly marked. The pit is
situated such that it straddles a small ridge that runs east-west. Mining has primarily occurred on the north facing side of the
ridge to date. Topsoil was properly stripped and stockpiled along the north permit boundary. The permit area was originally
laid out using the county road that runs along the south side of the area as a starting point and the layout appeared to coincide
with the mine plan map. However, sometime after the mine was started, the State Land Board commissioned a survey that
seems to conflict with the previously identified property boundaries. The result is that the original lay-out appears to be
slightly out of sink with this over-laid survey. The perimeter of the permit boundary was walked and the size of the pit is
correct. Although this is not being cited as a problem, the operator should probably re-survey the permit boundary to ensure its
accuracy for their own information. This concluded the inspection.
I & E Contact Address
NAME: Curtis Stone
OPERATOR: Fremont County
STREET: 615 Macon Ave.. Rm. 102
CITY/STATE/ZIP: Canon City.
