S~ <br />i-' <br />•b <br />r <br />-~ <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTIOCi DATE 3-7-00 INSPECTOR'S INITIALS BK_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 2-29-00. Present during the <br />inspection were Miae Lana Wilson, a student at the School of Mines and Scott Lewis of CC&V. <br />The main purpose of the inspection was to give Miss Wilson a site visit of a valley leach <br />facility. She requested the site visit to assist her in the project she ie working on, <br />regarding environmental impacts associated with such facility. She wanted to see the safe <br />guards the operator has put in place, to minimize impacts to the environment associated with <br />such an operation. Mr Scott Lewis, Manager Environmental affairs for the operation, gave a <br />brief history of the operation After the briefing, the physical inspection began around the <br />American Eagle look out area where Mr. Lewis pointed out the locations of the sites he <br />covered during the briefing. Inspected the Iron Clad Globe hill waste storage areas, hey were <br />active at the time of the inspection. Inspected the upper Squaw Gulch Waste storage area. <br />Mr.Lewie explained what the sequence of the placement of the waste rock in the area and the <br />storm water erosional protection that is in place for the area. Inspected the main Creeaoa <br />pit. It was not active, at the time of the inspectioa. Inspected the Crusher area. Mr Lewis <br />explained the sequence that takes place, with the crusher. Crusher was not active at the time <br />of the inspection. Run of mine material, which dose not require crushing was being placed on <br />to the pad at the time of the inspection. Inspected Phase 2, II & III leach pad area. Mr <br />Lewis gave a brief presentation of the sequence of leaching sad how solution from the pad is <br />transported into the process plant. Inspected the uader drain system for the leach pad <br />facility. A brief preseatation was provided to on the design of the under drains. The <br />southeast under drain and the 4" pipes were discharging at the time of the inspection.
