r <br />';, ~, <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 2-2-00 <br />(Page 2) <br />INSPECTOR'S INITIALS BK <br />The Division conducted an inspection of the Phase II and Phase III leach pad expansion areas <br />while on site for a meeting with CC&V and Golder and associates representatives about the <br />pending Amendment #8. <br />Inspected the Phase II, leach pad area, the third lift in last SUMMER'S expanded area is in <br />place. The west facing slopes were covered with approximately 2" of snow. Inspected the Phase <br />III area, approximately 3 feet of cemented rock backfill was in place, it is expected that <br />the rest of the cemented rock back fill and to be in place with in the next few days. In <br />fact, according to Mr. Lewis, all remediation work should be completed in the area, with in <br />the next few weeks. <br />Inspected the Phase II pregnant solution collection system area and recorded the following <br />readings from the High and Low Solution Collection system transducers. <br />HIGH VOLUME SOLUTION PUMP #4 READ 19.4 FEET OF HEAD, #5 READ 15.4 FEET OF HEAD AND # 6 READ <br />18.8 FEET OF HEAD.PUMP #4 STILL READS HIGHER THAN #6, WHICH USED TO NORMALLY READ HIGHER. <br />LOW VOLUME SOLUTION PUMP # 1 READ .43 FEET OF HEAD AND #2 READ .16 FEET OF HEAD. <br />Inspected the Cresson pit, it was active, run of mine material was being placed on to the <br />leach pads at the time of the inspection. No major problem noted. Inspected the Squaw Gulch <br />Waste Storage area, it was active at the time of the inspection. Drainage control in the <br />area, appears to be in good repair. <br />After the physical inspection, a meeting was held with Scott Lewis, Ron Roberts and Terry <br />Mandziac representing CC&V John Luppo and Tim Cazier representing Golder and associates <br />the engineering firm who will be working on Amendment #8 The main purpose of the meeting <br />was to understand the investigative work that has been conducted to date in determining the <br />extent of the underground workings in the pending Amendment #8 area. John Lupo explained the <br />findings to date and what additional work that Golder will conduct prior to finalizing the <br />remedial plan for each working. During the discussion, the Division recommend's that since <br />the Leak detection system under the present facility is not functioning as designed, to <br />completely line the trench with geomembrane in the pending Amendment #8 area.
