ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_Ieal_ns;n <br />INSPECTION DATE: L/.l I/nd INSPECTOROS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site at the request <br />of the County and as part of sites to be inspected every four years. The <br />site is a small sand and gravel operation/ quarry that utilizes blasting. <br />The blasting is conducted by MSHA certified subcontractors, on as need <br />basis, not by the operator. No explosive materials are stored on site. <br />Present during the inspection were Kathleen Sullivan of DMG and Russell Van <br />Krause, the operator who is conducting the mining operation and who is also <br />in the process of succeeding the operation from Howard and Barbara Stone, <br />the permitee. <br />The site is located approximately 10 miles South of Florissant Colorado at <br />an elevation of 8300 feet. With the exception of the pit floors, most of <br />the slopes were covered with snow. When the site was permitted in 1991, the <br />financial warranty was set at $1,800 by the MLRD Board. Since that time the <br />disturbance was very minimum. However during this inspection, there was <br />significant disturbance noted. According to the operator, the area was <br />recently surveyed and he said the affected area was approximately 3.8 <br />acres. Normally, a measuring wheel will be utilized to confirm the affected <br />acreage. However, since the operator is in the process of taking over the <br />permit and had the permit area surveyed recently, the Division requested <br />the operator to provide the new surveyed map, with contour lines, of the <br />entire permit area. In the map include the total affected acreage and <br />height and length of the small high walls noted at the time of the <br />inspection. Operator was also requested to place permit boundary markers <br />rather than the natural features such as trees that were allowed in the <br />past as permit boundary markers for the site. Operator agreed to place <br />markers at each corner of the permit boundary, prior to succeeding the <br />operation. Once the new survey map is submitted, the Division will <br />recalculate the financial warranty for the affected and for what is being <br />proposed to be affected in the coming year. <br />The entire affected area was walked, there were a few processed stockpile <br />material noted on the pit floor. There were two crushers, a screen, a small <br />dozer and a large dozer located along the Northwest side of the permit <br />area. Top soil/ overburden stockpiles appeared to be properly protected. <br />Permit sign posted. <br />
