(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1998-047 <br />INSPECTION DATE 07114/2006 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS DEG <br />This routine monitoring inspection of Permit Number M-1998-047 (Mantle Pit #3) was conducted by Deb E. GOnima of the <br />DMG. Moffat County representative Marvin Moore was present during this inspection. The pit is located approximately 16 miles <br />northeast of Dinosaur on County Road 16, and currently consists of approximately 17.9 acres where approximately 11 acres <br />have been disturbed. <br />Mine entrance signs and permit boundary markers are all in place, and topsoil stockpiles are all marked. However, some of the <br />topsoil stockpile marker signs, while in place, were damaged and may need to be replaced. All activities antl completed work at <br />the time of inspection appeared to follow the guidelines and regulations set forth by the state and the mining and reclamation <br />plans for the pit, and no problems are noted for this site. The site is stable and no disturbance was observed outside of the <br />approved permit boundaries. For future reference, this site is correctly positioned in the DMG online GIS. <br />The large material stockpile is situated near the northern-center of the site, and south of the pit (see photo 1). The majority of <br />the site is still active, and no reclamation has begun. The pit area is in the shape of a basin and lies at the far northern portion <br />of the site (see photo 4). Well-marked topsoil stockpiles are located to the west of the material stockpile at the northern portion <br />of the site (see photo 2). At the time of this inspection there was a propane tank and a grader on site (as seen in photos 1 and <br />2). These items appeared to bewell-maintained and did not appear to be hazardous. The majority of the undisturbed area lies <br />at the eastern portion of the site (see photo 3). <br />A storm water runoff control program was implemented a few years back and seems to have been successful in its use of <br />diversion structures, sediment traps, and small ponding areas (see photo 3). There appears to be no offsite erosion, and no <br />long-term (greater than 72 hours) water retention was observed. <br />Responses to this inspection report should be directed to Deb E.
